NFZ Health Insurance for Foreign Workers in Poland: A Practical Guide
Working in Poland as a foreign worker? If your employer pays your ZUS social security contributions, you're entitled to free healthcare through Poland's National Health Fund (NFZ — Narodowy Fundusz Zdrowia). The challenge is that few people explain how the system actually works in practice. This step-by-step guide shows you how to use your NFZ coverage day-to-day.
Who qualifies for NFZ coverage?
You have the right to NFZ healthcare if:
- You're employed on an employment contract (umowa o pracę) or a civil-law contract (umowa zlecenia) and your employer pays your health insurance contribution to ZUS
- You hold a Temporary Residence Card (Karta Czasowego Pobytu) or another valid legal residence permit in Poland
- You're registered as unemployed at a local employment office (Urząd Pracy)
Important: Having a work permit alone does not give you NFZ access — you need an active contract with contributions being paid. Confirm with your employer that your contributions are being submitted regularly.
How to find an NFZ doctor
Your first step is choosing a primary care physician (POZ — Podstawowa Opieka Zdrowotna), also called a "family doctor" (lekarz rodzinny). This is your main doctor who issues referrals to specialists.
How to find an NFZ primary care doctor:
- Visit znaleźlekarz.nfz.gov.pl or open the mojeIKP app
- Enter your city and filter by "NFZ"
- Choose a clinic or doctor with available appointments
- Call or visit in person to submit a Declaration of Choice of POZ Doctor (a simple form — your name, PESEL number, signature)
You can change your primary care doctor once per year for free (more often in exceptional circumstances).
What to bring to your appointment
For your first visit, bring:
- A photo ID (passport or national ID card)
- Your PESEL number (required for registration)
- Your residence card (if you have one)
The receptionist will verify your insurance status in the EWUŚ system (Electronic Verification of Healthcare Entitlements). If the system doesn't confirm your coverage, you can sign a statement of entitlement to benefits — in this case, the doctor is legally obligated to see you.
Getting a referral to a specialist
For most specialists under NFZ, you'll need a referral from your POZ doctor. Exceptions — you can go directly, without a referral, to:
- Psychiatrist
- Dentist (though the scope of free services is limited)
- Gynecologist and obstetrician
- Ophthalmologist
- Dermatologist
- Venereologist
- Oncologist
Your referral is valid at any NFZ specialist clinic in Poland — you don't have to use the one nearest to you.
NFZ at the dentist
NFZ covers only basic dental services: a consultation, tooth extraction, amalgam filling, and tartar removal once per year. Cosmetic services (white composite fillings, veneers, orthodontics) are not covered and must be paid privately. Many dentists have an NFZ contract for only some services — ask before your appointment what's included.
Emergency services and SOR — when to go where
For urgent medical needs, there are three channels:
- Ambulance: 112 or 999 — life-threatening emergencies (heart attack, accident, loss of consciousness)
- SOR (Hospital Emergency Department) — urgent but not immediately life-threatening situations that require fast diagnosis or treatment
- NiŻ (Night and Weekend Healthcare) — evenings, nights, and weekends when your POZ is closed, for non-emergency situations (sore throat, ear pain, mild infection)
If you're insured by NFZ, emergency and SOR care is free of charge — even if the EWUŚ system temporarily can't confirm your coverage.
Prescription drug reimbursement
Prescription medications may be partially or fully reimbursed by NFZ: free (0 PLN), at 30%, or at 50% of the full price. Your doctor marks whether a drug is reimbursable when writing the prescription. At the pharmacy, ask for a "reimbursed medication" (lek refundowany) or a cheaper generic equivalent.
The mojeIKP app — highly recommended
Download the mojeIKP app (My Online Patient Account) — available on App Store and Google Play. After logging in with your Trusted Profile (Profil Zaufany), you can:
- Check your current NFZ insurance status
- View your history of appointments, test results, and prescriptions
- Download your e-ZLA (electronic sick leave certificate)
- Book appointments online (at clinics with e-registration)
What to do if NFZ care is denied
If a medical facility refuses to provide NFZ-covered care without justification:
- Request a written refusal
- Contact the NFZ branch office for your voivodeship (region) — contact details at nfz.gov.pl
- You can also file a complaint with the Patient Rights Ombudsman (free helpline: 800 190 590)
